Planned Gift Notification Form

You can honor the Hollywood Theatre in your estate planning in a variety of ways. Listed below are some of the most common arrangements.

Charitable Bequests
Your intent to make a bequest to the Hollywood Theatre can be made clear in your will or living trust using simple and clear language, upon consultation with your attorney. You can stipulate a gift of a specific dollar amount, a percentage of the value of your estate, or property such as stock.

Sample Bequest Language:

“I give, devise and bequeath to the Hollywood Theatre, a non-profit organization (Tax ID #93-1074861) with a principal business address of 4035 NE Sandy Blvd, Suite 212, Portland, OR 97212, for its general purposes, all [or state the fraction or percentage] of the rest, residue, and remainder of my estate, both real and personal.”

Qualified Charitable Distributions (QCDs)
Qualified Charitable Distributions (QCDs), also known as IRA Charitable Rollovers, are the savviest way for individuals age 70½ or older to use their IRAs to maximize their charitable impact. This generous, tax-wise gift will help you make the most of your legacy while supporting the Hollywood Theatre now.

Stock Gifts
Donating a charitable gift of stock or mutual fund shares is a great way to support the mission of the Hollywood Theatre and receive an end-of-the-year tax break. Stock or mutual fund donations are particularly attractive if they have increased in value since the beginning of this calendar year—you will receive a charitable deduction for the value of the donated shares, and you will not have to pay any capital gains tax on the increased value of the donated shares. Retirement Plans
IRAs, 401(k)s, 403(b)s, Keoghs, and other retirement plan assets make up an increasingly large part of many estates. When left to your heirs, these assets can be subject to income, estate, and other transfer taxes. Naming the Hollywood Theatre as a beneficiary of all or a percentage of your plan may reduce or eliminate these taxes.

Life Insurance
If you have a life insurance policy that has outlived its original purpose (e.g., to provide security to minor children), consider gifting this policy to the Hollywood Theatre. Gifting a life insurance policy can greatly reduce your taxable estate. Gifting a policy can also yield a current income tax deduction of the policy's fair market value, which can sometimes be significant. You may also simply name the Hollywood Theatre as a beneficiary of your policy.

Memorial Gifts
Many individuals request that survivors send charitable gifts in lieu of flowers. You can request that your friends and loved ones send such memorial gifts to the Hollywood Theatre, 4035 NE Sandy Blvd, Suite 212, Portland, OR 97212. Please ask them to indicate who is being honored, as well as who should be notified of the memorial gift.
